Kindergarten Registration
Kindergarten Registration for all Habersham County Elementary Schools will be held at the Habersham 9th Grade
Academy (across from Demorest Elementary at Georgia Highway 365 and Demorest-Mt. Airy Highway.)
A child must be five years old by September 1, 2008 to enroll in kindergarten.
We encourage parents to bring their children on the assigned date for the school district in which they reside. However, parents may bring their children to any of the registrations.
Registration Dates are as follows:
Tuesday March 25, 2008 Baldwin and Demorest 1:00 P.M. – 6:00 P.M.
Wednesday March 26, 2008 Clarkesville and Fairview 1:00 P.M. – 6:00 P.M.
Thursday March 27, 2008 Cornelia and Woodville 1:00 P.M. – 6:00 P.M.
Friday March, 28, 2008 Level Grove and Hazel Grove 1:00 P.M. – 6:00 P.M.
Parents must supply to school officials the following:
1. A Valid Birth Certificate with the seal, file number, and signature of the registrar
2. Georgia Certificate of Immunization - Form 3231
Please bring your child’s current immunization form 3231.
If your child needs immunizations, Habersham County Health Department personnel will be available during all registrations to administer immunizations. Shots are $14.00 each. Payment is due at the time
of service. Peachcare, Medicaid, personal checks or cash will be accepted.
3. The Child's Social Security Card
4. Proof of 911 Address (Example: Power Bill)
5. Certificate of Eye, Ear and Dental Examinations - Form 3300
There are two ways to obtain this form.
1. Children will be given an eye, ear and dental screening during registration. Form 3300 will then be completed for their files.
OR
2. If your child has already been given these examinations at the Habersham County Health Department or at their physician’s office you may bring the completed form 3300 to registration.
The Lollipop Assessment will be administered by a kindergarten teacher during registration. It is very important that you bring your child for this process so that plans can be made for your child's entrance into kindergarten. Be sure to allow 2 hours for the complete registration process. Parents will accompany their children throughout the registration process.
